Accessing Your Information

As laid out in the Privacy Policy, you have the right to view your information. You can also request a copy of your records verbally or in writing (e.g. through the Practice Better app).

Viewing Your Record During an Appointment

During an appointment, you can request to see exactly what is in your record. The dietitian will either verbally read what is is in the record or share her screen so you can see the record yourself, depending on the specific nature of the information and request.

Getting a Copy of Your Records

You may also request a copy of the records held by Samantha Holmgren Dietetic Services.

This copy can be sent as an electronic version or a paper copy mailed to you. We reserve the right to charge a nominal fee:

• Digital version (encrypted) $30 or,
• Paper version (printed and mailed) $30 for the first 20 pages and $0.25 per page after.

Record Maintenance

Samantha Holmgren Dietetic Services will retain a copy of your records for ten (10) years after your last appointment or ten (10) years after your eighteenth (18th) birthday if your last appointment occurred before you turned eighteen (18) years of age. This is the minimum length required by the College of Dietitians of Ontario. After this time, the record will be destroyed.

Correcting Your Record

If you notice any factual errors (e.g. wrong birthdate, address, etc), simply inform the dietitian about these errors and they will be corrected.

If you disagree with any other information in the record, discuss this with the dietitian. Depending on the information in question, it may not be changed. However, you have the right to file a statement of disagreement which will be included in your health record as well.
